Start the computer and hold down the shift key to start with the extensions off.

After you see "Welcome to Macintosh Extensions Off".

Hold down the command and option keys to rebuild the desktop.

At the prompt:

"Are you sure you want to rebuild the desktop"   Select OK.

When finished restart the computer so all extensions will be enabled.
